4^th step - sending samples to be tested

P-TEST contains everything needed for taking and sending samples by mail to GENERI BIOTECH. After collection of samples, you will have 2 or 3 tubes with samples on brushes. Make a payment of 4.950 CZK (approx. 195 €) either by post coupon or bank transfer or using our website PAYMENT GATEWAY. You have to fill out the same name on the post coupon as the sender did on the SAMPLE RECORD. If you make a bank transfer or a payment through PAYMENT GATEWAY you will find detailed instructions in the brochure.

1) fill in the SAMPLE RECORD form that is contained in the brochure

2) put the following into the return envelope which is included in this kit:

3) send the envelope with samples as soon as possible, by current mail, registered mail or by an express service (FedEx, DHL or other services available in your country). If you do not send it out immediately, put the samples into a refrigerator until sent out.
